Core Responsibilities
Primary Functions
- Provide personalized assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and oral hygiene, while respecting client dignity.
- Assist clients with mobility, including safe transfers from bed to chair, walking assistance, and support with positioning to prevent discomfort.
- Offer genuine companionship and emotional support through active listening, engaging conversation, and participating in hobbies and activities.
- Prepare and serve nutritious, well-balanced meals and snacks tailored to the client's specific dietary needs, preferences, and restrictions.
- Administer consistent and accurate medication reminders to ensure clients adhere to their prescribed medication schedules.
- Support clients with toileting and incontinence care in a sensitive, respectful, and hygienic manner.
- Monitor vital signs, such as blood pressure and temperature, as directed by a healthcare professional and report any significant changes.
- Encourage and assist with prescribed physical therapy exercises, light stretching, and short walks to maintain mobility and strength.
- Foster a safe, clean, and healthy living environment by performing light housekeeping duties, including laundry, dishwashing, and tidying up living spaces.
- Observe, document, and promptly report any changes in the client's physical condition, mental state, or behavior to the supervising nurse and family.
- Engage clients in mentally stimulating activities like reading, playing games, doing puzzles, and reminiscing to promote cognitive health.
- Provide palliative or hospice support, focusing on comfort, pain management, and emotional well-being during end-of-life stages.
Secondary Functions
- Accompany clients to medical appointments, social outings, and other activities, providing transportation and support.
- Run essential errands for the client, such as grocery shopping, picking up prescriptions, and mailing packages.
- Maintain detailed, accurate, and timely daily care logs, documenting services provided, client's condition, and any incidents.
- Communicate effectively and professionally with the client's family, providing updates on their well-being and collaborating on the care plan.
- Assist with the use and maintenance of medical and adaptive equipment, such as walkers, wheelchairs, and hearing aids.
- Implement and uphold the client's personalized care plan as developed by the care management team.
- Manage the client's daily schedule, including appointments, mealtimes, and social activities, to create a structured and comfortable routine.
- Promote the client's independence and self-esteem by encouraging them to do as much for themselves as safely possible.
- Respect and maintain client confidentiality and privacy in all aspects of care and communication.
Required Skills & Competencies
Hard Skills (Technical)
- CPR and First Aid Certification: Current certification is highly preferred and often required.
- Safe Patient Handling: Proficiency in body mechanics for lifting, transferring, and repositioning clients to prevent injury.

Minimum Education:

Experience Requirements
Typical Experience Range: 0 - 5+ years. While professional experience is a plus, personal experience caring for a loved one is also highly valued.
Preferred: 1+ years of documented experience providing care in a professional setting such as a home care agency, assisted living facility, or nursing home. A valid driver's license and reliable transportation are often required.
