Core Responsibilities

Primary Functions

  • Conduct comprehensive, strengths-based assessments of family dynamics, needs, and challenges in both home and community settings.
  • Develop, implement, and continuously review dynamic and collaborative Individualized Service Plans (ISPs) in partnership with each family.
  • Skillfully connect families with a wide network of community resources, including housing assistance, food security programs, healthcare services, employment opportunities, and mental health support.
  • Provide immediate and effective crisis intervention, de-escalation, and safety planning for families experiencing acute distress.
  • Serve as a tenacious advocate for clients, ensuring their rights are protected and their voices are heard within complex systems like schools, courts, and healthcare.
  • Develop and maintain a strong, trusting, and empathetic rapport with a diverse caseload of families, honoring their unique cultural backgrounds and experiences.
  • Facilitate productive family team meetings, case conferences, and mediations to foster communication and collaborative problem-solving.
  • Monitor, document, and evaluate client progress toward their goals, adjusting service plans as necessary to ensure continued momentum and success.
  • Empower clients by providing education and coaching on essential life skills, including financial literacy, effective parenting techniques, and navigating social services.
  • Conduct regular home visits to assess for safety, offer hands-on support, and strengthen the case manager-family relationship.
  • Assist clients with the practical tasks of securing support, such as completing applications for benefits, services, and housing.
  • Manage a caseload of 15-25 families, providing consistent and high-quality support tailored to each family's unique situation.
  • Provide direct transportation for clients and their children to essential appointments, such as medical visits, court dates, and school meetings, to overcome logistical barriers.
  • Prepare for and, when necessary, provide clear and professional testimony in court proceedings related to child welfare and family well-being.
  • Utilize trauma-informed care principles in all interactions to create a safe and healing-centered environment for clients.

Secondary Functions

  • Maintain meticulous, accurate, and confidential client records, case notes, and service documentation in compliance with agency standards and legal requirements.
  • Compile and submit timely statistical reports, outcome data, and other required documentation for program evaluation and funding purposes.
  • Actively participate in weekly team meetings, clinical supervision sessions, and professional development trainings to enhance skills and knowledge.
  • Build and nurture a robust network of community partners and referral sources through proactive outreach and collaboration.
  • Contribute to the organization's mission by participating in agency-wide events, committees, and strategic initiatives.
  • Support ad-hoc data requests and exploratory data analysis to identify trends in client needs.
  • Contribute to the organization's data strategy and roadmap by providing frontline feedback on data collection tools.
  • Collaborate with business units to translate data needs into engineering requirements.
  • Participate in sprint planning and agile ceremonies within the data engineering team.

Required Skills & Competencies

Hard Skills (Technical)

  • Case Management Software: Proficiency in using Electronic Health Record (EHR) or client management database systems for documentation and reporting.
  • Resource Knowledge: Deep and current knowledge of local, state, and federal community resources, social service programs, and eligibility requirements.
  • Legal & Regulatory Compliance: Understanding of HIPAA, child welfare laws, and mandatory reporting requirements.
  • Driving: Valid driver’s license, a clean driving record, and access to reliable, insured transportation.
  • Bilingual Fluency: Proficiency in a second language (especially Spanish) is highly desirable to serve diverse communities.

Education & Experience

Educational Background

Minimum Education:

  • A Bachelor’s degree from an accredited college or university.

Preferred Education:

  • A Master's degree in Social Work (MSW), Counseling, or a related human services field.

Experience Requirements
