Core Responsibilities

Primary Functions

  • Provide comprehensive, patient-centered, and continuous primary medical care to individuals and families across all ages, from infants to the elderly.
  • Diagnose and effectively manage a wide spectrum of acute illnesses, chronic diseases, and complex health conditions such as diabetes, hypertension, asthma, and heart disease.
  • Conduct thorough routine physical examinations, preventative health screenings, and annual wellness visits, tailoring recommendations to individual patient risk factors.
  • Develop, implement, and meticulously monitor personalized, evidence-based treatment plans in collaboration with patients to achieve shared health goals.
  • Order, perform, and accurately interpret a variety of diagnostic tests and procedures, including laboratory work, imaging studies, and EKGs, to aid in diagnosis and management.
  • Perform a range of minor in-office surgical and medical procedures, such as suturing lacerations, skin biopsies, cryotherapy, joint injections, and IUD/Nexplanon placements.
  • Prescribe and manage patient medications, ensuring appropriateness, monitoring for efficacy and adverse effects, and educating patients on proper usage.
  • Counsel patients and their families on crucial topics including disease prevention, health maintenance, lifestyle modifications, nutrition, and fitness.
  • Build and maintain a dedicated patient panel, fostering long-term, trusted relationships and ensuring exceptional continuity of care.
  • Collaborate seamlessly with a multidisciplinary team including specialists, nurse practitioners, physician assistants, nurses, and medical assistants to coordinate holistic patient care.
  • Make timely and appropriate referrals to medical specialists, surgeons, and mental health professionals, ensuring a closed loop of communication and follow-up.
  • Maintain exceptionally accurate, detailed, and timely medical documentation in the Electronic Health Record (EHR) system in compliance with all legal and professional standards.
  • Adhere strictly to established clinical protocols, evidence-based guidelines, and organizational standards of care to ensure patient safety and quality.
  • Participate equitably in the on-call schedule to provide after-hours telephonic or clinical coverage for urgent patient needs.
  • Address patients' mental and emotional health concerns with empathy, providing initial counseling and appropriate referrals to behavioral health services.
  • Actively engage in continuous medical education (CME) and professional development to stay current with the latest medical advancements and best practices.
  • Manage care for patients in various settings, which may include the outpatient clinic, hospital, and/or long-term care facilities, depending on the practice structure.
  • Champion population health initiatives by identifying at-risk patients and implementing strategies to improve health outcomes for the community.

  • Actively participate in peer review processes, quality assurance committees, and practice improvement meetings to enhance clinical quality and patient safety.
  • Serve as a clinical mentor and preceptor for medical students, residents, and advanced practice providers (APPs), fostering the next generation of healthcare professionals.
  • Contribute to the development, review, and implementation of clinical policies, procedures, and workflows to optimize practice efficiency and patient care.
  • Engage in community health fairs, educational seminars, and other outreach programs to promote public health and wellness.
  • Support practice administration with tasks related to compliance, accreditation, and meeting value-based care metrics (e.g., HEDIS, MIPS).

Required Skills & Competencies

Hard Skills (Technical)

  • EHR/EMR Proficiency: Advanced proficiency in using Electronic Health Record systems such as Epic, Cerner, AthenaHealth, or eClinicalWorks for charting, ordering, and communication.
  • Broad Diagnostic Acumen: Expertise in diagnosing and treating a wide spectrum of common and complex medical conditions across all age groups.
  • Outpatient Procedural Skills: Competence in performing common office-based procedures (e.g., skin biopsies, joint injections, cryotherapy, basic suturing, IUD/Nexplanon insertion/removal).

Education & Experience

Educational Background

Minimum Education:

  • Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school.
  • Successful completion of a three-year ACGME or AOA-accredited Family Medicine Residency program.

Preferred Education:

  • Current Board Certification in Family Medicine by the American Board of Family Medicine (ABFM) or the American Osteopathic Board of Family Physicians (AOBFP).
  • Fellowship training in a relevant subspecialty (e.g., Sports Medicine, Geriatrics, Obstetrics, Palliative Care) is a plus.

Experience Requirements
