Core Responsibilities
Primary Functions
- Conduct comprehensive physical assessments and detailed health histories for patients of all ages, from infants to geriatric individuals, to establish baseline health and identify concerns.
- Independently diagnose and manage common acute illnesses, injuries, and infections by developing and implementing evidence-based treatment plans.
- Oversee the long-term management of chronic health conditions such as diabetes, hypertension, asthma, and COPD through regular monitoring, medication adjustments, and ongoing patient support.
- Order, perform, and interpret a wide range of diagnostic tests, including laboratory work, EKGs, and imaging studies, to aid in diagnosis and treatment planning.
- Prescribe appropriate pharmacological agents, treatments, and non-pharmacological therapies based on patient diagnosis, clinical guidelines, and state-specific prescriptive authority.
- Perform various in-office procedures including, but not limited to, immunizations, wound care, suturing, skin biopsies, and incision and drainage.
- Deliver comprehensive preventative care and health screenings, including well-child checks, annual physicals, and women's health exams (e.g., Pap smears, contraception counseling).
- Formulate, implement, and evaluate patient-centered care plans in close collaboration with patients, their families, and the interdisciplinary healthcare team.
- Provide clear and effective education to patients and families regarding disease prevention, health promotion, and the management of their specific health conditions to empower self-care.
- Counsel patients on crucial lifestyle modifications, including diet, exercise, smoking cessation, and stress management, to improve overall health outcomes and reduce risk factors.
- Identify the need for specialty care and facilitate appropriate referrals to specialists, ensuring seamless care coordination and communication across the healthcare continuum.
- Meticulously and accurately document all patient encounters, including assessments, diagnoses, treatments, and follow-up plans, within the Electronic Health Record (EHR) system.
- Manage a dedicated panel of patients, fostering long-term, trusting therapeutic relationships and ensuring continuity of care.
- Respond to patient telephone calls, portal messages, and other electronic communications, providing timely medical advice and addressing concerns within the scope of practice.
- Assess and triage patients presenting with urgent or emergent needs, efficiently determining the appropriate level of care and course of action.
Secondary Functions
- Collaborate effectively with a multidisciplinary team including physicians, medical assistants, and administrative staff to optimize clinic workflow and enhance the patient experience.
- Actively participate in regular team meetings, huddles, and case conferences to discuss complex patient cases and coordinate comprehensive care strategies.

Education & Experience
Educational Background
Minimum Education:
- For RN roles: Bachelor of Science in Nursing (BSN) from an accredited institution.
- For NP roles: Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program.
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- For NP roles, must hold National Board Certification as a Family Nurse Practitioner (FNP-BC or FNP-C).
Preferred Education:

Experience Requirements
Typical Experience Range: 2-5 years of clinical nursing experience, preferably in a primary care, ambulatory, or community health setting. New graduate FNPs with strong clinical rotation experience in family medicine are encouraged to apply.
