Core Responsibilities

Primary Functions

Provide direct assistance to patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and oral hygiene, while promoting patient independence.
Accurately measure and record patient vital signs, such as temperature, blood pressure, pulse, respiration rates, and oxygen saturation, reporting any significant deviations to the supervising nurse immediately.
Meticulously observe patients' physical, mental, and emotional conditions, promptly reporting any changes, symptoms, or patient concerns to the nursing staff to ensure timely intervention.
Safely assist patients with mobility, including transferring from bed to chair, ambulation, turning, and repositioning, utilizing proper body mechanics and assistive devices like gait belts and patient lifts.
Proactively ensure patient safety by maintaining a hazard-free environment, ensuring call lights are within reach, using bed alarms as indicated, and implementing fall prevention protocols.
Perform and document basic diagnostic procedures as delegated, such as point-of-care blood glucose testing, and collect various specimens including urine, stool, and sputum for laboratory analysis.
Assist patients with their nutritional and hydration needs by preparing meal trays, feeding or assisting with feeding, monitoring food and fluid intake, and documenting consumption accurately.
Maintain precise and timely documentation of all care provided, observations, and patient interactions within the Electronic Health Record (EHR) or patient chart, adhering to facility standards.
Respond promptly and courteously to patient call lights and requests, anticipating needs to enhance patient comfort and satisfaction.
Assist nursing staff with basic treatments and procedures, such as performing non-sterile dressing changes, applying skin barrier creams, and assisting with catheter care.
Reinforce patient and family education that has been provided by the nursing or therapy staff, clarifying instructions on mobility, diet, and self-care activities.
Create and maintain a clean, safe, and organized patient environment by changing bed linens, clearing clutter from rooms, and ensuring equipment is stored properly.
Assist with the admission, transfer, and discharge processes, including orienting patients to their room, packing/unpacking belongings, and transporting them as needed.
Provide compassionate emotional support and companionship to patients and their families, fostering a therapeutic and respectful rapport.
Apply, monitor, and document the use of protective devices or restraints as ordered by a physician and under the direct supervision of a nurse.

Assist with the stocking of patient rooms and nursing stations with essential medical supplies and linens.
