Core Responsibilities
Primary Functions
- Serve as the primary point of contact and advocate for patients, guiding them through the complexities of the healthcare and social services systems.
- Conduct comprehensive, culturally sensitive assessments to identify patient barriers to care, including transportation, financial constraints, housing instability, food insecurity, and health literacy.
- Develop and implement individualized care plans to address identified barriers, connecting patients with appropriate internal and external community resources.
