Core Responsibilities

Primary Functions

Lead the design and execution of comprehensive Community Health Needs Assessments (CHNA) to identify key health priorities, service gaps, and underlying social determinants of health.
Analyze and interpret complex quantitative and qualitative data from various sources (e.g., epidemiological, demographic, clinical) to identify health trends, disparities, and priorities.
Develop, implement, and monitor long-range, evidence-based strategic health plans and implementation strategies in collaboration with community partners.
