Core Responsibilities
Primary Functions
- Initiate, monitor, and discontinue hemodialysis treatments according to physician orders and facility protocols, ensuring patient stability and treatment efficacy.
- Perform and document comprehensive pre- and post-treatment patient assessments, including obtaining vital signs, monitoring blood pressure, and measuring weight to calculate fluid loss.
- Expertly cannulate arteriovenous (AV) fistulas and grafts using aseptic technique, and access central venous catheters (CVCs) for dialysis treatment, while monitoring for signs of complications.
- Set up, prime, and conduct safety checks on hemodialysis machines and associated equipment, including dialyzers and bloodlines, ensuring all parameters are correct before initiating treatment.
- Respond promptly and effectively to patient complications during treatment, such as hypotension, cramping, and clotting, by making appropriate machine adjustments and notifying the registered nurse.
- Accurately calculate and adjust fluid removal rates (ultrafiltration) based on the patient's pre-weight, target weight, and fluid intake.
- Administer prescribed medications such as heparin, lidocaine, and saline under the direction and supervision of a licensed registered nurse.
- Meticulously document all aspects of the dialysis treatment, including machine parameters, patient observations, medications administered, and any interventions, in the electronic medical record (EMR).
- Reinforce patient and family education on critical topics including diet, fluid restrictions, medication adherence, and vascular access care to promote patient self-management and positive outcomes.
- Ensure patient comfort and safety throughout the entire 3-4 hour treatment process, providing emotional support and addressing individual patient needs with empathy and professionalism.
- Collect, properly label, and prepare patient blood samples for routine and ad-hoc laboratory analysis, ensuring sample integrity and timely transport.
- Continuously monitor patient vital signs and all machine parameters (e.g., blood flow, pressures, conductivity) throughout the treatment, reporting any deviations or alarms to the charge nurse.
- Inspect the patient's vascular access pre-treatment for signs of infection, stenosis, or thrombosis and report findings to the nurse.
- Foster a positive and supportive environment by building therapeutic relationships with patients who require long-term, chronic care, recognizing their unique physical and emotional challenges.
Secondary Functions
- Perform and document water quality testing, including checks for chlorine, chloramine, and hardness, to ensure the water used for dialysis meets AAMI standards.
- Conduct thorough cleaning, disinfection, and sterilization of dialysis machines, patient chairs, and other equipment between treatments to prevent cross-contamination and maintain infection control standards.
- Manage and maintain inventory levels of critical dialysis supplies, including dialyzers, needles, and solutions, by stocking stations and reporting shortages.
- Actively participate in interdisciplinary team meetings, contributing observations about patient status and collaborating with nurses, dietitians, and social workers to optimize the patient's plan of care.
- Adhere strictly to all organizational policies and procedures, as well as federal regulations regarding infection control, hazardous materials, patient safety, and HIPAA.
- Assist with the orientation, training, and precepting of new patient care technicians and students, sharing knowledge and demonstrating best practices.
- Evaluate and troubleshoot technical issues with hemodialysis machines and water treatment systems, performing minor repairs or escalating to biomedical engineering as needed.
- Ensure the treatment area is maintained in a clean, safe, and orderly manner before, during, and after each shift to ensure a professional and safe clinical environment.
