Core Responsibilities

Primary Functions

  • Provide compassionate and respectful assistance with personal care routines, including bathing, showering, dressing, grooming, and toileting, while prioritizing client dignity and comfort.
  • Assist clients with mobility and transfers, utilizing proper body mechanics and safety devices like gait belts or lifts to help them move safely around their home.
  • Plan, prepare, and serve nutritious meals that align with the client's specific dietary requirements, health needs, and personal preferences.
  • Administer timely medication reminders to ensure clients adhere to their prescribed medication schedules as directed by their healthcare provider.
  • Engage clients in meaningful social interaction and companionship, such as conversation, reading aloud, playing games, or participating in hobbies to combat loneliness and promote mental stimulation.
  • Perform light housekeeping duties to maintain a clean, safe, and healthy living environment, including laundry, washing dishes, vacuuming, and tidying up common areas.
  • Accompany clients to medical appointments, errands, grocery shopping, and social outings, providing transportation and support as needed.
  • Monitor and observe the client's physical and mental condition, promptly reporting any significant changes in health, behavior, or needs to the supervising nurse or agency manager.
  • Assist with prescribed therapeutic exercises or physical therapy routines as instructed by a healthcare professional to support the client's recovery or maintenance of mobility.
  • Provide essential support and respite for family caregivers, offering them a break while ensuring their loved one receives professional and reliable care.
  • Help clients manage their schedules and appointments, providing reminders for doctor's visits, social events, and other important commitments.
  • Create and maintain a supportive and positive atmosphere that promotes the client's overall well-being and emotional health.

Secondary Functions

  • Maintain accurate, timely, and detailed records of the care provided, including daily activities, client responses, and any incidents, in accordance with agency policy.
  • Communicate effectively and professionally with the client's family members, providing updates on their well-being and collaborating on the care plan.
  • Ensure all safety protocols and infection control procedures are strictly followed to protect both the client and yourself.
  • Manage household needs such as grocery lists, tracking household supplies, and organizing mail for the client.
  • Provide specialized care for clients with chronic conditions such as dementia, Alzheimer's, or diabetes, following specific protocols and a personalized care plan.
  • Respond calmly and effectively to emergency situations, following established procedures and contacting emergency services when necessary.

Educational Background
