Core Responsibilities

Primary Functions

Provides direct, hands-on personal care to patients, including bathing, dressing, grooming, oral hygiene, and toileting, always prioritizing patient comfort and dignity.
Assists patients with safe mobility, transfers, and ambulation, proficiently using proper body mechanics and assistive devices such as gait belts and Hoyer lifts.
Performs essential light housekeeping tasks necessary for the patient's health and safety, such as changing bed linens, tidying the immediate patient area, and laundering patient's personal items.
Prepares and serves light meals and provides feeding assistance as required, ensuring adherence to the patient's dietary plan and preferences.
Accurately observes, records, and reports any changes in the patient's physical condition, functional abilities, behavior, or emotional state to the supervising RN Case Manager.
Measures and documents vital signs, including temperature, pulse, respiration, and blood pressure, as outlined in the patient's individualized plan of care.
Offers compassionate companionship and crucial emotional support to patients and their families, fostering a calm, reassuring, and respectful environment.
Follows the established, patient-specific plan of care meticulously as developed and supervised by the Registered Nurse.
Documents all care provided, tasks performed, and patient observations accurately and in a timely manner within the agency's electronic medical record (EMR) system.
Reinforces patient and family education on personal care techniques, safety precautions, and various aspects of the care plan as directed by the RN.
Maintains a clean, safe, and healthy environment for the patient, consistently implementing infection control protocols and universal precautions.
Provides respectful and sensitive post-mortem care for the deceased patient while offering immediate emotional support to the grieving family.
Communicates effectively and professionally with all members of the interdisciplinary team, including nurses, social workers, and chaplains, to ensure holistic and coordinated patient care.
Manages and maintains an inventory of necessary medical supplies, personal care items, and equipment in the patient's home, reporting needs to the clinical team.
Recognizes and immediately reports any signs of patient abuse, neglect, exploitation, or unsafe conditions to the appropriate supervisor and authorities.
Provides essential respite for family caregivers, allowing them personal time while ensuring the patient continues to receive high-quality, uninterrupted care.
Actively participates in interdisciplinary team meetings to contribute to the review and update of patient care plans.
Travels to patient homes or residential care facilities, managing time and logistics efficiently to meet a schedule of daily visits.

Education & Experience

Educational Background

Minimum Education:

High School Diploma or GED.
Successful completion of a state-approved Certified Nursing Assistant (CNA) or Home Health Aide (HHA) training program.
Must hold an active, unencumbered certification/licensure as a CNA or HHA in the state of practice.

Preferred Education:
