Core Responsibilities

Primary Functions

  • Provide comprehensive assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, always ensuring the client's dignity and comfort.
  • Offer dedicated mobility support, which includes safe transferring from bed to wheelchair, assisting with walking using aids, and consistent repositioning to prevent pressure sores and maintain skin integrity.
  • Plan, prepare, and serve nutritious and appealing meals that strictly adhere to the client's specific dietary requirements, medical restrictions, and personal preferences.
  • Meticulously manage and administer medication reminders to ensure the client follows their prescribed medication schedule with precision and without error.
  • Deliver genuine companionship and crucial emotional support by engaging in meaningful conversations, sharing in hobbies, playing games, and accompanying the client on walks or outings.
  • Monitor the client’s physical and mental condition, vigilantly observing for any changes in health, mood, or behavior and reporting these observations to family and healthcare professionals.
  • Provide overnight supervision and care as needed, responding to nighttime awakenings, emergencies, or requests for assistance to ensure client safety around the clock.
  • Implement and support care plans developed by the client's medical team, including nurses, doctors, and therapists, to ensure a cohesive approach to care.
  • Respond promptly and calmly to medical emergencies, demonstrating knowledge of emergency protocols and knowing when to contact emergency services.
  • Assist with prescribed therapeutic exercises, such as physical or occupational therapy routines, to support the client's rehabilitation and functional maintenance goals.
  • Foster a positive, encouraging, and safe environment that promotes the client's sense of independence, self-worth, and overall well-being.

Secondary Functions

  • Perform light housekeeping tasks to maintain a clean, organized, and hygienic living space, including doing laundry, tidying rooms, washing dishes, and changing bed linens.
  • Manage all grocery shopping and other essential errands, such as pharmacy pickups, to ensure the home is consistently stocked with necessary food, supplies, and medications.
  • Coordinate and provide reliable transportation to medical appointments, physical therapy sessions, social gatherings, and other community-based activities.
  • Maintain a detailed daily log or journal documenting care activities, medication intake, meal consumption, and any significant observations or incidents.
  • Act as a primary communication liaison between the client, their family, and their extended healthcare team, providing regular and clear updates.
  • Manage the household calendar, including scheduling and tracking medical appointments, social visits, and other important events.
  • Ensure the client’s safety at all times by identifying and mitigating potential household hazards and maintaining a clear and accessible living space.
  • Assist with the proper use and maintenance of medical equipment such as oxygen tanks, nebulizers, or lift devices, after receiving appropriate training.
