Core Responsibilities

Primary Functions

  • Serve as the central point of contact for patient care coordination, meticulously scheduling appointments, complex procedures, and essential follow-up visits to ensure a seamless patient journey.
  • Act as a dedicated liaison between patients, their families, physicians, nurses, and other healthcare providers to facilitate clear communication and collaborative care.

  • Meticulously verify patient insurance eligibility and benefits, and proactively obtain pre-authorizations and pre-certifications for treatments, procedures, and hospital admissions.
  • Maintain and update electronic medical records (EMR/EHR) with the utmost accuracy and confidentiality, ensuring all patient information, clinical notes, and test results are correctly filed.
  • Educate patients and their families on their care plans, explaining pre-operative and post-operative instructions, medication schedules, and next steps in a clear and compassionate manner.
  • Skillfully manage inbound and outbound patient referrals, ensuring a smooth transition of care and the timely transfer of all necessary medical documentation between facilities.
  • Uphold strict adherence to HIPAA regulations and other healthcare privacy laws, safeguarding patient confidentiality in all communications and record-handling.
  • Address and resolve patient inquiries, concerns, and complaints with empathy and professionalism, escalating complex issues to the appropriate clinical or administrative leadership.
  • Triage patient phone calls and electronic messages, assessing urgency and directing them to the appropriate physician, nurse, or clinical staff member for prompt attention.
  • Coordinate all aspects of hospital admissions and discharges, including arranging for post-discharge services like home healthcare, physical therapy, and durable medical equipment (DME).
  • Ensure all necessary medical charts, lab results, and imaging reports are collected, organized, and available for review by the clinical team prior to patient appointments.
  • Prepare and maintain diverse clinical documentation, including patient consents, treatment summaries, and administrative reports required for operational and compliance purposes.
  • Proactively manage provider schedules to maximize clinic efficiency, minimize patient wait times, and accommodate urgent or add-on appointments.
  • Provide high-level administrative support to a team of physicians, specialists, and other healthcare providers, anticipating needs and facilitating a smooth workflow.
  • Conduct systematic follow-up calls and communications with patients post-procedure or post-discharge to monitor recovery, answer questions, and reinforce care instructions.
  • Oversee the accurate collection and entry of patient demographic, financial, and insurance information during the registration and intake process.
  • Facilitate communication among the multidisciplinary care team through huddles, meetings, and secure messaging to ensure everyone is aligned on the patient's treatment plan.
  • Process and manage requests for medical records from patients, other healthcare facilities, and insurance companies in a timely and compliant manner.
