Core Responsibilities

Primary Functions

  • Accompany the healthcare provider into patient examination rooms to comprehensively document the patient's history, physical examination findings, and clinical progress in the Electronic Health Record (EHR) in real-time.
  • Meticulously transcribe the entirety of the patient encounter, capturing the History of Present Illness (HPI), Review of Systems (ROS), Past Medical, Surgical, and Family History, Physical Exam (PE), and Assessment and Plan (A&P).
  • Accurately record physician-dictated diagnoses, prescriptions, lab tests, imaging studies, and treatment plans, ensuring complete clarity for medical billing and future patient care.
  • Facilitate efficient patient flow by anticipating provider needs, tracking the status of lab results and imaging studies, and preparing charts for upcoming appointments to minimize patient wait times.
  • Input and manage orders for laboratory tests, imaging, and medications as directed by the attending physician, verifying correctness and clinical appropriateness before submission in the EHR.
  • Locate and retrieve historical patient data, including previous encounter notes, lab results, and specialist reports from the EHR or other sources for provider review during the visit.
  • Prepare and update patient charts prior to appointments, ensuring all relevant information, recent test results, and required documentation are readily available for an efficient visit.
  • Assist in generating referral letters, consultation requests, prior authorizations, and other clinical correspondence to specialists and other healthcare entities.
  • Document all consultations and discussions with referring physicians, specialists, and family members, maintaining a complete and chronological record of care coordination activities.
  • Ensure all clinical documentation is fully compliant with institutional policies, medicolegal standards, and HIPAA regulations to protect patient privacy and data integrity.
  • Support the medical coding process by accurately documenting all procedures performed and diagnoses made, using appropriate medical terminology to justify reimbursement.
  • Scribe for a wide variety of medical procedures, from routine office visits to complex specialty consultations or emergency department interventions, adapting documentation style as needed.
  • Prepare and print patient instructions, discharge summaries, and work/school excuse notes as dictated by the physician, ensuring the patient leaves with clear follow-up directives.
  • Manage and triage incoming clinical messages and patient portal inquiries, summarizing relevant information for the provider to review and address in a timely manner.
  • Act as a vital communication link between the physician, patient, and other healthcare team members, ensuring clarity, accuracy, and continuity of care.
  • Proactively monitor documentation for and alert the physician to any discrepancies, inconsistencies, or missing information within the patient's chart.
  • Check on the progress and results of X-rays, CT scans, and other imaging studies, communicating the status and preliminary reads to the physician upon availability.
  • Respond to messages from the clinical care team and front desk, processing requests for information or prescription refills under the direct supervision of the provider.
  • Navigate complex Electronic Health Record (EHR) systems with high proficiency to quickly access and input information, maximizing provider efficiency and reducing screen time.
  • Chart patient progress notes, procedure notes, and formal medical reports with exceptional accuracy and attention to detail.

Secondary Functions

  • Assist with the training and onboarding of new medical scribes, providing mentorship and guidance on documentation best practices and provider-specific workflows.
  • Help maintain and organize clinic resources, including educational handouts and standardized documentation templates within the EHR.
  • Participate in team huddles and departmental meetings to contribute to process improvement and enhance the overall efficiency of the clinical workflow.
  • Support administrative tasks as needed, such as faxing records, making follow-up calls to patients or pharmacies, and preparing paperwork under provider direction.

Required Skills & Competencies

Hard Skills (Technical)

  • Medical Terminology: Deep understanding of medical terms, anatomy, physiology, and pharmacology.
  • EMR/EHR Proficiency: Experience with or ability to quickly master electronic health record systems (e.g., Epic, Cerner, AthenaHealth, eClinicalWorks).
  • Typing Speed and Accuracy: A minimum typing speed of 60-65 words per minute with high accuracy is essential.

Education & Experience

Educational Background

Minimum Education:

  • High school diploma or equivalent (GED).
  • Completion of a medical scribe training program or relevant certification may be required.

Preferred Education:

  • Bachelor’s degree in a science-related field or current enrollment in a pre-health professional track (pre-med, pre-PA, pre-nursing).
