Core Responsibilities
Primary Functions
- Conduct comprehensive patient assessments at the beginning of the shift and continually monitor patient status, vital signs, and neurological condition throughout the night.
- Administer prescribed medications, including oral, intravenous, subcutaneous, and intramuscular injections, while adhering to the five rights of medication administration.
- Execute and document physician's orders accurately and in a timely manner, ensuring all treatments and interventions are carried out as planned.
- Provide expert wound care, including dressing changes, wound vac management, and assessment for signs of infection or delayed healing.
- Manage intravenous (IV) lines, including insertion, maintenance of IV fluids, and monitoring for infiltration or phlebitis.
- Perform and interpret diagnostic tests at the bedside, such as blood glucose monitoring, and collect specimens for laboratory analysis.
- Respond promptly and effectively to patient calls, alarms, and emergent situations, initiating life-saving interventions and CPR when necessary.
- Maintain meticulous and real-time documentation in the Electronic Health Record (EHR/EMR) system, chronicling all patient care activities, observations, and responses to treatment.
- Develop, implement, and update individualized patient care plans in collaboration with the interdisciplinary team.
- Educate patients and their families on their condition, medications, and discharge instructions, ensuring they are prepared for ongoing care.
- Safely transfer and reposition patients to prevent pressure ulcers and ensure comfort, utilizing proper body mechanics and assistive devices.
- Monitor and manage medical equipment, such as cardiac monitors, ventilators, and infusion pumps, troubleshooting issues as they arise.
- Provide a detailed and concise handoff report to the incoming day shift nursing staff, ensuring seamless continuity of care.
- Act as a patient advocate, ensuring their rights are respected and their physical and emotional needs are met during the night shift.
- Supervise and delegate tasks to nursing assistants (CNAs) and other ancillary staff, ensuring the delivery of high-quality care.
- Collaborate with night-shift physicians, respiratory therapists, and other healthcare professionals to coordinate comprehensive patient care.
- Identify and assess changes in patient condition, escalating critical findings to the appropriate physician or rapid response team.
- Provide comfort and emotional support to patients and their families, especially during times of distress or anxiety.
- Ensure a safe and clean patient environment, adhering to all infection control policies and procedures.
- Participate in rounding on patients to assess for pain, positioning, and personal needs throughout the overnight hours.
- Manage patient admissions, transfers, and discharges that occur during the night shift, completing all necessary documentation and procedures.
- Initiate and maintain patient restraints or seclusion protocols as per facility policy and physician orders, ensuring patient safety.
Secondary Functions
- Participate in unit-based quality improvement initiatives and performance improvement projects.
- Serve as a preceptor or mentor for new graduate nurses and newly hired nursing staff on the night shift.
- Contribute to staff meetings and engage in shared governance committees to improve nursing practice and patient outcomes.
- Assist with maintaining inventory of medical supplies and equipment on the unit, reporting any shortages or malfunctions.
Required Skills & Competencies
Hard Skills (Technical)
- Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) Certification

- Skilled in intravenous (IV) therapy, including insertion and management of peripheral and central lines

Educational Background
Minimum Education:
- Associate's Degree in Nursing (ADN) from an accredited institution.
- Active and unencumbered Registered Nurse (RN) license in the state of practice.

Experience Requirements
