Core Responsibilities

Primary Functions

  • Perform and document comprehensive patient assessments, including the regular monitoring, interpretation, and reporting of vital signs (blood pressure, heart rate, temperature, respirations, oxygen saturation).
  • Assist patients with Activities of Daily Living (ADLs), such as bathing, grooming, dressing, ambulation, and feeding, while promoting patient independence and dignity.
  • Administer prescribed medications and treatments as delegated by and under the direct supervision of the supervising Registered Nurse, ensuring adherence to the "five rights" of medication administration.
  • Observe, document, and promptly report any changes in a patient's physical, mental, or emotional condition to the charge nurse and primary RN.
  • Provide direct patient care, including wound dressing changes, catheter care, and glucose monitoring, following established protocols and sterile techniques.
  • Develop foundational critical thinking skills by actively participating in care planning and collaborating with the RN to prioritize patient needs on the night shift.
  • Ensure a safe patient environment by conducting regular rounds, responding promptly to call lights, and implementing fall prevention and safety protocols.
  • Accurately and thoroughly document all care provided, patient observations, and interventions in the Electronic Health Record (EHR) in a timely manner.
  • Communicate effectively and compassionately with patients and their families, providing emotional support and basic education regarding their care plan.
  • Assist with the admission, transfer, and discharge processes, including patient orientation to the unit, inventory of personal belongings, and coordination of transport.
  • Collaborate with the interdisciplinary team, including physicians, therapists, and support staff, to ensure cohesive and comprehensive patient care.
  • Participate actively in shift hand-off reports, ensuring a seamless and safe transition of care between outgoing and incoming nursing staff.
  • Reinforce patient and family education on disease processes, treatment plans, and post-discharge instructions as initiated by the Registered Nurse.
  • Manage and maintain medical equipment, ensuring it is clean, in proper working order, and readily available for patient care needs.
  • Uphold strict adherence to all hospital policies, procedures, and infection control standards to maintain a safe and sterile environment for all.

Secondary Functions

  • Assist in maintaining adequate stocking of medical supplies, linens, and nourishment in patient rooms and on the unit.
  • Participate in unit-based quality improvement initiatives and data collection efforts as assigned by nursing leadership.
  • Shadow Registered Nurses during complex procedures and critical patient events to enhance clinical understanding and learning.
  • Attend mandatory in-service training sessions and educational workshops to further develop professional nursing competencies.
  • Contribute to a positive and collaborative work environment by supporting team members and demonstrating a commitment to teamwork.
  • Engage in self-reflection and seek regular feedback from preceptors and mentors to identify learning needs and track progress toward professional goals.

Required Skills & Competencies

Hard Skills (Technical)

  • Basic Life Support (BLS) Certification: Current certification from the American Heart Association (AHA) or equivalent is required.
  • Vital Signs Assessment: Proficiency in accurately measuring and interpreting blood pressure, pulse, respirations, temperature, and oxygen saturation.
  • Medical Terminology: Strong working knowledge of common medical terms, abbreviations, and disease processes.
  • Electronic Health Record (EHR) Navigation: Basic computer skills and the ability to learn and effectively document within a hospital's EHR system.
  • Patient Care Fundamentals: Competency in performing basic nursing tasks such as patient hygiene, mobility assistance, and feeding.
  • Infection Control Principles: Solid understanding and application of standard precautions, hand hygiene, and personal protective equipment (PPE) usage.
  • Phlebotomy and IV Skills: Foundational knowledge or willingness to learn venipuncture and basic IV line management under supervision.

Education & Experience

Educational Background

Minimum Education:

  • Must be currently enrolled and in good academic standing in an accredited School of Nursing (ADN or BSN program).
  • Must have successfully completed at least one semester of clinical rotation in a hospital or acute care setting.

Preferred Education:

  • Currently enrolled in a Bachelor of Science in Nursing (BSN) program.
  • Recent graduate of an accredited nursing program who is eligible to take the NCLEX-RN exam.

Experience Requirements

Typical Experience Range: 0 - 1 year of clinical experience. This is an entry-level, developmental role.

Preferred:

  • Completion of a Med-Surg or other acute care clinical rotation.
  • Previous experience in a healthcare setting as a Certified Nursing Assistant (CNA), Patient Care Technician (PCT), or similar role.
