Core Responsibilities
Primary Functions
- Administer comprehensive daily hygiene and comfort care, including full or partial bed baths, showers, and oral hygiene, adapting techniques to the patient's level of dependency and health status.
- Assist patients with mobility, transfers, and positioning using appropriate ergonomic techniques and lifting equipment to ensure both patient and caregiver safety and to prevent pressure ulcers.
- Support patients with nutrition and hydration by assisting with meal preparation and feeding, monitoring food and fluid intake, and promptly reporting any changes in appetite or swallowing difficulties.
- Accurately measure, record, and monitor vital signs, including temperature, pulse, respiration rate, blood pressure, and pain levels, promptly reporting any significant deviations to the nursing staff.
- Collaborate closely with the registered nurse in the execution of the care plan, providing crucial feedback on patient response to treatments and contributing to care plan adjustments.
- Maintain a clean, safe, and welcoming environment for the patient, which includes making beds, changing linens, and ensuring the patient's room is tidy and free of hazards.
- Provide attentive listening and psychological support to patients and their families, fostering a trusting and therapeutic relationship while communicating with empathy and respect.
- Observe and report any physical, behavioral, or psychological changes in the patient's condition to the supervising nurse in a timely, clear, and precise manner.

- Facilitate effective communication for patients with sensory, cognitive, or speech impairments, using adapted communication tools and person-centered techniques.
- Respond promptly and courteously to patient call bells and inquiries, addressing their needs efficiently or escalating to the appropriate team member when necessary.
- Engage in continuous professional development and mandatory training to stay current with best practices in patient care, safety protocols, and new healthcare technologies.
- Support patient rehabilitation efforts by encouraging and assisting with prescribed exercises and activities of daily living as guided by physical or occupational therapists.
