Core Responsibilities

Primary Functions

  1. Patient & Family Education: Serve as the primary educator for transplant candidates, living donors, and their families, clearly explaining the evaluation process, surgical procedures, risks, benefits, alternative treatments, and post-transplant lifestyle requirements.
  2. Comprehensive Clinical Assessment: Conduct thorough pre-transplant evaluations of potential recipients and living donors, including detailed medical history reviews, physical assessments, and coordination of diagnostic testing.
  3. Waitlist Management: Meticulously manage and maintain the organ transplant waitlist in strict accordance with United Network for Organ Sharing (UNOS)/Organ Procurement and Transplantation Network (OPTN) policies, ensuring accurate data entry and timely updates.
  4. Interdisciplinary Team Coordination: Facilitate seamless communication and collaboration between all members of the transplant team, including surgeons, physicians, social workers, dietitians, financial coordinators, and pharmacists.
  5. Regulatory Compliance & Data Submission: Ensure all clinical and administrative activities are in full compliance with UNOS/OPTN, CMS, and Joint Commission standards, and be responsible for the accurate and timely submission of all required data forms.
  6. On-Call Responsibilities: Participate in a rotating on-call schedule to manage organ offers, coordinate emergent transplant surgeries, and triage urgent patient issues outside of standard business hours, often under significant time pressure.
  7. Organ Offer Coordination: Evaluate and coordinate deceased donor organ offers, communicating critical clinical information to the transplant surgeon and medical team to facilitate a timely decision to accept or decline.
  8. Surgical & Logistical Scheduling: Coordinate the complex scheduling of transplant surgeries, including operating room time, staffing, and all pre-operative patient preparations and admissions.
  9. Inpatient Care Coordination: Round on hospitalized pre- and post-transplant patients, ensuring the plan of care is being followed, providing patient education, and facilitating a smooth transition to the outpatient setting.
  10. Post-Transplant Monitoring: Closely monitor and manage the care of patients following transplantation, including tracking lab results, managing immunosuppressive medications in collaboration with providers, and identifying early signs of rejection or infection.
  11. Living Donor Advocacy: Act as a dedicated advocate for living donors, ensuring their safety, autonomy, and well-being throughout the evaluation, donation, and recovery process, separate from the recipient's care.
  12. Protocol & Policy Development: Assist in the development, implementation, and refinement of clinical protocols, patient care policies, and standard operating procedures for the transplant program.
  13. Financial Coordination Liaison: Collaborate with financial coordinators to ensure patients understand the financial aspects of transplantation and have the necessary insurance authorizations and resources in place.
  14. Outpatient Clinic Management: Organize and participate in outpatient transplant clinics, ensuring patients are seen in a timely manner and that all aspects of their ongoing care plan are addressed.
  15. Documentation Excellence: Maintain impeccable, thorough, and confidential patient records within the Electronic Health Record (EHR) system, documenting all interactions, clinical decisions, and patient progress.
  16. Quality Improvement Participation: Actively participate in the program's Quality Assurance and Performance Improvement (QAPI) initiatives by collecting data, identifying trends, and contributing to process improvement projects.
  17. Community & Professional Outreach: Represent the transplant program at hospital-wide meetings, community events, or professional conferences to promote awareness and education about organ donation and transplantation.
  18. Triage of Patient Communications: Serve as the first point of contact for patient calls, expertly triaging clinical concerns, medication questions, and other issues to the appropriate team member.
  19. Discharge Planning: Develop and implement comprehensive discharge plans for post-transplant patients, ensuring they are equipped for self-management at home with appropriate medications, follow-up appointments, and support systems.
  20. Management of Immunosuppression: Under physician-approved protocols, manage and adjust complex immunosuppressive medication regimens based on lab values, drug levels, and patient's clinical status.
  21. Psychosocial Support Coordination: Work closely with social workers and psychologists to assess the psychosocial stability of candidates and ensure they have adequate support systems to cope with the stresses of transplantation.
  22. Transition of Care: Coordinate the long-term transfer of care back to the patient's referring physician when clinically appropriate, providing detailed summaries and guidelines for ongoing management.

Secondary Functions

  • Support and contribute to transplant-related clinical research studies, including patient screening and data collection.
  • Contribute to the continuous improvement of transplant program protocols and patient care pathways.
  • Mentor and precept new transplant coordinators, nurses, or other clinical staff joining the team.
  • Participate in sprint planning and agile ceremonies for technology or process improvement projects within the transplant department.

Required Skills & Competencies

Hard Skills (Technical)

  • UNOS/UNet Proficiency: In-depth knowledge and hands-on experience with UNet data entry, waitlist management, and compliance requirements.
  • EHR/EMR Expertise: High proficiency in using Electronic Health Record systems such as Epic, Cerner, or Allscripts for charting, order entry, and data retrieval.

Experience Requirements

Typical Experience Range: 3-5+ years of direct patient care experience as a Registered Nurse.

  • Experience in a critical care setting (ICU/CCU), nephrology, hepatology, or complex case management is highly desirable.
