Core Responsibilities

Primary Functions

  • Conduct comprehensive health and lifestyle assessments using validated tools (health risk assessments, biometric screening results, readiness-to-change questionnaires) to establish baseline metrics and individualized goals for each client.
  • Develop and implement personalized, evidence-based wellness plans that integrate nutrition guidance, physical activity recommendations, stress reduction techniques, sleep hygiene, and behavior-change strategies tailored to client preferences and medical history.
  • Use motivational interviewing and behavior-change counseling techniques to empower clients, elicit intrinsic motivation, and set SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals that promote long-term adherence and measurable outcomes.
  • Deliver recurring one-on-one coaching sessions in-person and via telehealth platforms (video conferencing and secure messaging), adapting communication style to adult learning preferences and health literacy levels.
  • Facilitate group workshops, webinars, and corporate wellness classes on topics such as weight management, tobacco cessation, stress resilience, healthy eating on a budget, and ergonomics to drive population-level engagement.
  • Create and maintain clear, timely documentation of coaching encounters, progress notes, care plans, and outcome measures in electronic health records (EHR/EMR) or coaching platforms while ensuring HIPAA-compliant practices.
  • Monitor client progress through objective metrics (weight, blood pressure, HbA1c, activity minutes) and subjective reports, analyze trends, and adjust plans through data-informed coaching interventions.
  • Coordinate with primary care providers, dietitians, mental health professionals, and allied health team members to ensure continuity of care, appropriate referrals, and integrated support for clients with complex or chronic conditions.
  • Conduct routine biometric screenings and health risk assessments at worksites, community events, or clinical settings and interpret results to recommend evidence-based next steps or referrals.
  • Design and deliver corporate wellness programs and campaigns (challenges, preventive screenings, lunch-and-learns) aligned to organizational goals, employee demographics, and measurable KPIs such as participation, engagement, and health risk reduction.
  • Develop client-facing educational materials, handouts, digital content, and behavior-change toolkits that are culturally sensitive, inclusive, and accessible across languages and literacy levels.

  • Implement relapse-prevention strategies and crisis response protocols; identify signs of deteriorating mental or physical health and initiate emergency referrals or safety plans in collaboration with clinical partners.
  • Lead onboarding, training, and supervision for mentee coaches, interns, or peer-support volunteers to maintain quality and fidelity to program protocols and evidence-based coaching standards.
  • Track program outcomes and KPIs (retention rate, goal attainment, biometric improvements, customer satisfaction) and prepare regular reports for stakeholders to demonstrate impact and inform strategic planning.
  • Participate in continuous quality improvement initiatives by collecting client feedback, conducting program evaluations, and iterating content and delivery modalities to increase effectiveness and scalability.
  • Support client retention and revenue goals by maintaining high client engagement through proactive outreach, re-engagement strategies, and efficient scheduling and follow-up practices.

Required Skills & Competencies

Education & Experience

Educational Background

Minimum Education:

  • Associate degree in health-related field OR high school diploma with equivalent health coaching certification and relevant experience.

Preferred Education:

  • Bachelor’s degree in Nutrition, Kinesiology, Exercise Science, Nursing, Public Health, Counseling, Psychology, or related field.
  • Graduate degree (MPH, MS, or MEd) or advanced clinical credential is a plus.
